Your watch may soon warn you about a health problem that roughly 40% of adults don't know they have. On August 12, Google announced insulin resistance tracking for Pixel Watch and Fitbit Air. No needle. No glucose sensor. No blood draw.

That's a bold claim, so let's look at exactly what Google said, how the technology works under the hood, and what it can't do.

What Google announced

Insulin Resistance Trends is one of four features in Google's new Health Guardian suite, unveiled August 12 for Pixel Watch and Fitbit Air. Health Guardian runs quietly in the background, watches for subtle physiological shifts over weeks, and delivers a monthly summary in the Google Health app. If something needs your attention, you get a notification.

The four features:

  1. Insulin Resistance Trends tracks shifts in your metabolic health over multiple weeks, without a blood test
  2. Blood Pressure Trends estimates blood pressure patterns from pulse and motion, no cuff and no calibration
  3. Sleep Breathing Quality Trends gives you a morning readout of how much of your sleep was spent breathing optimally
  4. Breathing Emergency Detection (Pixel Watch, select European countries) is an opt-in safety feature that can call emergency services if your blood oxygen drops critically and you become unresponsive

Insulin resistance is the headline feature, and it's the one worth understanding.

How it works (there's no glucose sensor)

Let's clear up the biggest question first. The Pixel Watch does not have a hidden glucose sensor. Neither does the Fitbit Air. Nobody has cracked reliable non-invasive glucose measurement on a wrist, and Google is not claiming to have done it.

What the watch measures is the stuff it already collects: heart rate, movement, and sleep. A single heart rate reading tells you almost nothing about your metabolism. But several weeks of heart rate, sleep, and activity data can reveal broader physiological patterns, and insulin resistance leaves a footprint across those signals.

Google's model looks for that footprint. The feature is built on the company's large sensor foundation model, SensorFM, which was pretrained on over one trillion minutes of sensor data from 5 million opted-in users across more than 20 Fitbit and Pixel Watch models. Instead of experts manually labeling thousands of hours of readings, the model taught itself human biology by observing raw sensor streams at enormous scale. It can even fill in gaps when the device misses a beat or captures incomplete data.

Google validated the approach against standard clinical blood tests and published the results in Nature. It also ran a dedicated study, WEAR-ME, with 1,165 participants, comparing wearable-based predictions against conventional fasting glucose and insulin measurements.

The honest framing: the watch is not measuring your blood sugar. It's recognizing the trail that rising insulin resistance leaves behind elsewhere in your body.

What you'll actually see

This is not a score and not a glucose reading. Google describes it as trend detection, closer to a warning light than a lab result.

Here's the flow:

  • You need about 30 days of wear before Health Guardian delivers its first detailed summary, because it's looking for long-term patterns rather than reacting to single readings
  • The monthly summary shows your insulin resistance trend alongside blood pressure and sleep breathing information
  • If the trend looks concerning, you'll see a flag such as High
  • Google Health Coach can then explain what the system noticed and suggest next steps around activity, sleep, and nutrition
  • You can view daily details or monthly overviews

Google's pitch: these shifts happen quietly. Roughly 40% of adults are unaware their insulin resistance is changing. Catching the trend early gives you a shot at adjusting nutrition, sleep, and movement before it turns into a bigger metabolic problem.

Which watches get it, and when

This is not a Pixel Watch 5 exclusive. Google's comparison pages now list Insulin Resistance Trends as arriving in September on:

There's one catch: the feature sits behind Google Health Premium, so a compatible watch alone isn't enough.

The rest of Health Guardian deserves a look too

The insulin feature gets the headlines, but blood pressure tracking may matter more day to day. Six in 10 people don't realize when their blood pressure has shifted higher, and Google's version works passively from pulse and motion with no cuff and no manual calibration. It's built on a second foundation model called WavesFM, trained on billions of minutes of raw heart and movement data, and validated through a Fitbit Hypertension Labs study.

Sleep Breathing Quality Trends gives you a morning readout on how well you breathed overnight, rolled up into a monthly assessment.

Breathing Emergency Detection is the most dramatic of the four. If a breathing emergency causes a persistent, critical drop in oxygen saturation and you become unresponsive, the watch can automatically call emergency services and alert your emergency contacts. It's opt-in, and limited to select European countries at launch.

The honest caveats

  • It's a flag, not a diagnosis. A "High" trend is a reason to confirm with a blood test or talk to a doctor, not a medical conclusion
  • The consumer version is unproven. Google has published strong research, but we don't know yet how often the shipping feature will raise false alarms or miss real changes
  • It costs extra. Google Health Premium is required
  • It's slow by design. Thirty days of wear before the first summary means this won't react to one bad week of eating

The Bottom Line

Google is betting that weeks of ordinary sensor data, run through a model trained on a trillion minutes of human biology, can catch metabolic problems that blood tests usually find years later. The research backing is unusually strong for a wearable feature: a Nature publication, a 1,165-person validation study, and comparisons against gold-standard blood tests.
